Study Links GLP‑1 Use to Fewer Breast Cancer Diagnoses

Researchers at the University of Pennsylvania examined electronic health records of 94,827 women aged 45 to 80 who were overweight or obese and had undergone breast imaging from 2022 to 2025. After matching participants on demographics and health factors, they compared those prescribed a GLP‑1 medication with those who were not.

In the matched cohort, 1.65% of GLP‑1 users received a breast cancer diagnosis versus 2.20% of non‑users, indicating roughly a 25% reduction in odds. The findings were presented by study author Elizabeth McDonald, MD, PhD at the 2026 American Society of Clinical Oncology (ASCO) Annual Meeting (Abstract 10506).

Dr. McDonald noted that breast cancer risk rises with age and that many women gain weight during perimenopause and menopause, but she emphasized that observational studies can be biased. “Only a prospective clinical trial can show whether these drugs truly prevent disease,” she said, adding that her team hopes to launch such a trial soon.

GLP‑1 Use Associated With Higher Ten‑Year Survival

A separate analysis published in JAMA Network Open reviewed records of 841,831 women diagnosed with stage I‑III breast cancer between 2006 and 2023. Using a matching method, the study compared outcomes for GLP‑1 users and similar patients who did not take the drugs.

Among women with obesity, the ten‑year survival probability was 96.0% for GLP‑1 users versus 88.6% for non‑users. For those with type 2 diabetes, survival was 96.9% for GLP‑1 users compared with 76.4% for patients on insulin or metformin.

In practice, the data imply that women who are already taking GLP‑1 drugs for obesity or diabetes might experience a modest advantage in cancer outcomes, but the degree of benefit remains uncertain.

Potential Mechanisms and Ongoing Questions

One hypothesis is that GLP‑1 drugs improve metabolic health, reducing the inflammatory environment that can promote tumor growth. Fat tissue produces estrogen and inflammatory chemicals, while high insulin levels in diabetes can stimulate pathways involved in cancer cell proliferation. By helping patients lose weight, control blood sugar, and lower inflammation, these medications may indirectly create a less favorable setting for cancer.

A second, less proven theory involves direct effects on tumor cells. Some breast cancers express GLP‑1 receptors, and early laboratory work suggests that targeting these receptors could impede tumor growth. However, clinical evidence for a direct anti‑cancer action is still lacking.

For women without obesity or diabetes, the current evidence does not support starting a GLP‑1 drug solely for cancer prevention. Dr. McDonald advises that such patients should use these medications only for FDA‑approved indications until results from randomized trials become available.

Patients already on GLP‑1 therapy at the time of a breast cancer diagnosis must decide with their care team whether to continue the medication during treatment. Some may tolerate the drug well, while others could experience worsened nausea or reduced appetite, potentially compromising nutrition during chemotherapy.

Overall, the emerging data highlight the importance of metabolic health in breast cancer outcomes, but definitive answers will require controlled clinical trials.
